Health Connections is the community outreach department at MedStar St. Mary's Hospital. It offers many community classes, seminars and support groups for all ages. Trained educators from Health Connections also visit area schools, community organizations, churches and business to speak about a variety of health and wellness topics. What is diabetes self-management?
Living with diabetes is a full-time job. The diabetes self-management classes at MedStar St. Mary's Hospital help individuals with diabetes take control of their disease. Our program is recognized by the American Diabetes Association and covers 10 areas of diabetes management from medications to emotions to daily activity to help individuals live better lives. Individuals interested in classes or nutrition counseling for diabetes can call 301-475-6019 for more information. These services do require an order from your physician.

I've just moved to the area and am eight months pregnant. Can I tour the hospital's Women's Health & Family Birthing Center?
For your convenience, MedStar St. Mary's Hospital has a virtual tour of our Women's Health & Family Birthing Center for expectant parents. The video can be accessed at MedStarHealth.org/StMarysVirtualTour. Childbirth classes are offered monthly through Health Connections. The class has a fee and registration is required. Is there information you can send to help us know what to expect when we arrive at the hospital?
Health Connections offers web-based and DVD options for childbirth classes. If you have additional questions, our staff of certified childbirth instructors and lactation consultants will be happy to talk with you over the phone. Call Health Connections at 301-475-6019 for more information.

Do you sell breast pumps?
The hospital's Breastfeeding Resource Center sells and rents breast pumps. As an official Medela distributor, we stock parts and support products. The center also sells baby slings, nursing bras, and supports. Our certified lactation consultants are available for individual consultations by phone, appointment, and at the weekly Breastfeeding Moms group held in the Health Connections building. Do you offer CPR and first aid classes?
Our Organizational Learning & Research Department offers two American Heart Saver CPR with AED classes and one American Safety Institute First Aid class each month. Because these classes are very popular, it's best to register early before your current certification expires.

What are Outpatient Care Coordinators?
Our care can extend beyond our beds at MedStar St. Mary’s Hospital. Many patients recently hospitalized or discharged from the Emergency Department are eligible for outpatient care coordination services to assist with at-home recovery. Outpatient Care Coordinators are Registered Nurses who provide guidance, support, and direction in managing health and wellness for patients after they leave the hospital. They have helped hundreds of people who suffer from multiple disorders, create plans to keep them safe, independent, and healthy in their own homes.
Outpatient Care Coordination is a free service and offers the ability to:
- Discuss your health concerns and answer questions
- Review your medications and discharge instructions with you
- Share information about ways to improve your health
- Work with you and your family to develop a personal health improvement plan
- Assist you with making follow-up appointments
- Identify and assist you with additional available resources
- Network with your primary care provider and healthcare team to ensure your healthcare needs are met
- Meet with you in your home, doctors’ office, or speak with you on the telephone

I need help with nutrition, do you have a dietitian on staff?
Yes! We have a registered dietitian on staff who offers a variety of counseling services on eating disorders and digestive issues, as well as food education sessions on how to better understand and manage your diet, tips on shopping at the supermarket or cooking at home, and resources for ingredient-specific recipes and products. Individuals must present a referral from your allergist or primary care physician. To make an appointment with a Health Connections dietitian, call 301-475-6019.

About the 340B Drug Pricing Program
The 340B Drug Pricing Program is a federal program that allows eligible hospitals, health centers, and clinics serving low-income and rural communities to purchase outpatient medications at significant, federally mandated discounts. Established by Congress in 1992, the program helps providers stretch limited resources and reinvest savings to improve access to care, support community health services, and address social factors that affect the health and well-being of underserved patients in the communities they serve.

340B Defined
The 340B Drug Pricing program is a bi-partisan program created by Congress in 1992. As defined by the U.S. Health Resources and Services Administration:
“The 340B Program enables covered entities to stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services.
Manufacturers participating in Medicaid agree to provide outpatient drugs to covered entities at significantly reduced prices.
[These savings are inherently not funded through taxpayer dollars, they are supported through required reduced pricing from drug manufacturers.]
Eligible healthcare organizations/covered entities are defined in statute and include HRSA-supported health centers and look-alikes, Ryan White clinics and State AIDS Drug Assistance programs, Medicare/Medicaid Disproportionate Share Hospitals, children’s hospitals, and other safety net providers.
To participate in the 340B Program, eligible organizations/covered entities must register and be enrolled with the 340B program and comply with all 340B Program requirements. Once enrolled, covered entities are assigned a 340B identification number that vendors verify before allowing an organization to purchase 340B discounted drugs”.

Supporting our Mission
The savings from MedStar St. Mary’s Hospital’s eligibility for 340B specifically supports our mission and allows us to advance programs and services that directly benefit the community. Although savings from the program are only a small fraction of our total community benefit, they are very meaningful. MedStar St. Mary’s Hospital’s savings through the 340B Program totaled $4.3 million in 2025, which is significantly less than the organization’s overall Community Benefit investment of $19.5 million during the same period. MedStar St. Mary’s Hospital has consistently allocated resources to programs and community support initiatives that far exceed the savings realized through the 340B Program.

Access to Care and Coordination of Care for Vulnerable Populations
MedStar St Mary’s Hospital is committed to ensuring our patients are able to safely and seamlessly return to care in the community upon leaving the hospital. MedStar St Mary’s has programs available to help those who may be more vulnerable due to their medical complexity, social situation, or lack of means to access needed services or medicines.
- The Transitional Care (TC) Program is a no-cost safety net service designed to support patients transitioning from hospital to home. Its primary goals are to improve patient outcomes, enhance communication and coordination among healthcare providers, and reduce the risk of patients declining after leaving the hospital. Trained nurses and social workers in the program proactively outreach patients at highest risk due to their medical and social complexity, providing structured telephonic- and technology-based support to ensure continuity of care between the hospital team and the patient’s doctors in the community. The clinical teams are assisted by Care Navigators, who provide real-time assistance scheduling all necessary follow-up appointments with existing providers or new specialists. Outreach occurs within 2 business days of discharge, and patients are followed for up to 30 days, receiving regular check-ins tailored to their needs.
- Caregiver Support Workshops & Resources
Support groups for patients and caregivers with chronic conditions such as Parkinson's disease and stroke are held monthly. MSMH participates in the Caregiver conference hosted each year by our local Department of Aging. - Mobile Health
MedStar St Mary’s has a 40 foot mobile unit that is available for health screenings, flu shot clinics and most recently as a satellite office for our Women’s Health service in our Community Benefit Service Area. The mobile unit is also available to make stops at our community hub in partnership one of our local soup kitchens and the Department of Social Services. - Behavioral Health Care Coordination
Patients discharged from our Behavioral Health Unit are offered a free 24/7 tech enabled care coordination and support program for the first 30 days post discharge provided by our partner, Mindoula. This program is designed to reduce utilization of the acute care system by assisting patients with long term support and resources in their home community.
Addressing Social Needs of Our Patients and Communities
MedStar St Mary’s Hospital provides screening to all our admitted patients for social needs impacting their health using our Social Needs Tool, and those who are identified as having needs for assistance are connected to numerous programs and partnerships to address the social needs of patients and community members. This includes access to our online directory of community social needs and resources, free or low-cost services encompassing assistance with medical care, healthy food, transportation, legal, utilities, and more.
- The Community Health Advocate (CHA) Program is a no-cost program to help patients with a particularly significant burden of social determinants impacting their health. This team, comprised of certified advocates recruited and trained from our community, provides direct patient assistance services, application assistance and referrals to community resources such as transportation, housing, health literacy, workforce development, and financial education. Advocates partner with patients as long as they need assistance, providing personal, relationship-based advocacy as they navigate a complex health environment.
- Transportation Assistance
In addition to taxi and bus fare assistance, MedStar St. Mary's Hospital provides a convenient rideshare program in partnership with Uber, giving a reliable means of transportation for vulnerable patients to medical appointments and health education activities. This service is coordinated by hospital staff and provided at no cost to the patient. MSMH also provides gap transportation for identified high risk patients to attend medical appointments in the early post discharge period.
Health & Wellness
MedStar St Mary’s delivers evidence-based, outcome-focused chronic disease management and prevention programs and services in its Community Benefit Service Area (CBSA).
- The Peer Recovery Coach (PRC) Program addresses the serious harm of opioid use disorder in our communities. Our emergency department offers universal patient screening and resources to patients via the nationally recognized best-practice Screening, Brief Interventions and Referrals to Treatment (SBIRT) approach. Those who are identified at highest risk for harm are connected with an onsite Peer Recovery Coach, a trained coach with personal experience in recovery from opioid addiction. Coach teams assist patients with accessing recovery services and engage throughout their journey to recovery in order to reduce the risk of relapse. They provide education and supplies to mitigate the risk of death from overdose.
- Fitness, Health & Wellness Programs
Community-based activities that promote physical activity, nutrition, stress management, and chronic disease prevention are available to our community members on a regular basis at low cost or free of charge to eligible patients. These programs focus on improving overall health outcomes and reducing long-term health issues. MedStar St. Mary's Hospital has designed a 6-week lifestyle management program called Healthy Next Steps to assist community members and their support persons with basic lifestyle management techniques to prevent and manage chronic conditions such as pre-diabetes, diabetes and high blood pressure. We also provide a monthly support group for diabetes. - Community-Wide Health & Wellness Events
Hospital-led community events such as health fairs, screening days, or awareness campaigns are part of a year-round calendar of offerings that increase access to health information, preventive services, and trusted clinical resources in community settings. - Maternal Wellness, Childbirth & Parenting Education Classes
Classes are available at our hospital to support expectant and new parents with evidence-based education on pregnancy, childbirth, infant care, and parenting, contributing to improved maternal and child health outcomes. These services are offered to new and expecting parents or grandparents in our community at low cost or free of charge to eligible patients. - Farmer’s Market
MSMH hosts a seasonal Farmer’s Market on our campus to provide an opportunity for those in the Leonardtown area to access fresh produce and other goods from local vendors. One of our vendors accepts SNAP and provides a new access point for SNAP participants to access fresh produce in the area.
Without the 340B program, sustaining these vital services would be a challenge—and many patients in our communities would lose access to the care they need.
Our commitment is simple: use every dollar saved through the 340B Program to improve health, expand access, and strengthen the communities we serve.

Members of the volunteer program at MedStar St. Mary's are an integral part of our long-standing reputation of excellence and quality care.
Each year, volunteers dedicate thousands of hours of service to facilitate the work of MedStar St. Mary's. By generously donating their time and talents, volunteers are able to meet new people, help the community, and explore healthcare careers.
At MedStar St. Mary's, we match volunteers to open positions based on individual interests, skills, talents, and availability. We ask that volunteers be willing to commit to at least six months of service. Our volunteers include adults, juniors, interfaith chaplains, and emergency drivers.
All individuals, without regard to race, color, national origin or ancestry, sex, religious affiliation, sexual orientation, marital status, veteran status, or physical or mental abilities, have a right to apply to MedStar St. Mary's for volunteer status, except for the specific age requirements stated. Volunteers are required to attend orientation and training sessions and agree to learn and abide by hospital regulations.
All volunteers are required to attend annual in-services on infection control; standard precautions; safety and security; hazardous and toxic substances; proper body mechanics; risk management; confidentiality; and customer service. All volunteers must also obtain a PPD test annually.
